If you are using an old tap that has been ground down to use as a punch wear a pair of gloves. Awhile doing exactly that I had a piece of it break off. The tiny little piece is still in the palm of my right hand . Been there for at least 25-30 years. I can still see the little spot where it is. If I press there I can feel it. Tried digging it out but never succeeded. Doctor didn’t want to mess with it. When using a tool I can still feel it in there at times.
