Anyone have a scan of the instruction sheet for the enclosed chain guard accessory?

I'd like to have it for a record regarding required hardware. Have one with I beleive all of the hardware. Also want to verify the plating on the hardware.
If a member bought the NOS one that just sold on Ebay and wants to share that information that would be great!!
